Student Questionnaire
At the start of the year, I ask the student to (voluntarily) complete a short list of questions about themselves, to help find out who they are, what their backgrounds are, and any other bits of information they think I should know. I then use this knowledge to modify any existing lesson plans, activities, and assignments to better accommodate the students in the course.
By introducing this to my practice, it has allowed me to start the year with a better understanding of each student. I complete the surveys and activities along with them, to show a willingness to be open with them. I think it helps to build a relationship of trust when I participate and am open about my experiences too.
In additition to the questionnaire, I also ask them to complete a couple of quizzes that are aimed at helping to determine your learning styles.
